Student Enrollment Form

I acknowledge that LTS’s NJTL programming may involve the use of program-arranged transportation to and from a nearby tennis facility. I understand that the program is not responsible for any delays or incidents that may occus during transport. I also understand that participation in LTS’s NJTL program involves physical activity that may result in injury. I release LTS, the contracted transportation provider, LTS staff, and volunteers from any liability, claims or demands that may arise from my child’s participation in the program, including but not limited to injury during training sessions, games, or transportation. I understand the risks involved and accept them on behalf of my student. I am also approving my student’s participation in periodic participation surveys that support LTS’s funding to implement its programs.
